Required Business Documents

 

The following information lists the documents required to open a business account at St. Mary's Bank:

  • St. Mary's Bank reserves the right to request additional documents if needed
  • Account name must match exactly to which the EIN has been assigned

Sole Proprietorship

  • Registered and in good standing with Secretary of State (SOS)
  • Social Security Number (SSN) *EIN is not required for account opening

General Partnership

  • Registered and in good standing with Secretary of State (SOS)
  • FINCen Certification of Beneficial Owner (UBO)
  • EIN from IRS
  • Certificate of Formation

Limited Liability Partnership

  • Registered and in good standing with Secretary of State (SOS)
  • FINCen Certification of Beneficial Owner (UBO)
  • EIN from IRS
  • Certificate of Formation

Limited Liability Corporation

  • Registered and in good standing with Secretary of State (SOS)
  • FINCen Certification of Beneficial Owner (UBO)
  • EIN from IRS
  •  Certificate of Formation

Corporation

  • Registered and in good standing with Secretary of State (SOS)
  • FINCen Certification of Beneficial Owner
  • EIN from IRS
  • Articles of Incorporation

IOLTA

  • FINCen Certification of Beneficial Owner (UBO)
  • IOLTA Authorization to Financial Institutions
  • Clear copy of Applicable Resolution (i.e. LLC, Partnership, Corp)

Non-Profit Corporation

  • Registered and in good standing with Secretary of State (SOS)
  • EIN from IRS

St. Mary's Bank does not open accounts for businesses conducting the following:

  • Money Transfer Business
  • Check Cashing Service
  • Virtual Currency
  • Marijuana Sales or Services (including medical)
  • Internet Gambling
  • Firearm Sales
  • Redeem/Sell Money Orders, Stored Value Cards, or Travelers Checks
  • ATM Operation/Ownership
  • Charitable Gaming - Bingo, Table Games, etc. (Including Non-Profit)
